  1. This application in no way obligates me to perform any volunteer services;
  2. The information I provided may be used to conduct a background check, to include driving records check, criminal background check, and other records where required by local, state, or federal law for volunteers working with youth;
  3. Big Brothers Big Sisters of Colorado (BBBSC) is not obligated to match me with a youth;
  4. As part of the BBBSC enrollment processes, I will be asked to provide additional personal information prior to making any recommendations for my assignment;
  5. Submission of this form is a portion of the volunteer enrollment process. Additional steps will be required to complete the volunteer enrollment process with Big Brothers Big Sisters of Colorado;
  6. I understand that there will be a $30.00 fee to cover expenses associated with processing your application. Additional background charges may be passed on to applicant. This fee is due at the time of interview.
